Personalized Care

There are many situations when in-home care is required and preferred. Our Director of Care will design a care plan for your loved one and ensure the right caregiver is matched to their personality and needs.

In-home care services are designed to help keep your loved one at home and give you the break you need so you can go back to being the daughter, son, or partner again.

We help with:
  • Meal preparation
  • Medication reminders
  • Light housekeeping and laundry
  • Personal care, such as grooming, toileting, bathing and dressing
  • Mobility and light exercises
  • Stimulating activities to keep the brain engaged
  • Companionship
  • Relief for the primary caregiver to focus on their own personal care or appointments

Top-Up Care

Often, the first step in getting extra support at home starts with a referral to your Local Health Integration Network or LHIN. This service typically provides you with a certain number of hours per week of government-funded caregiver support. While this support is a tremendous help, you may find that it is simply not enough.

This is where private healthcare companies, such as LiveWell Pathway, come into play. We provide top-up care to fill in the gaps. And unlike some private healthcare providers, we provide medical, non-medical and specialized clinical care options to our clients to meet your every need, from simple companionship to personal care to managing more complex clinical needs such as enteral feeds or colostomy bags.
